vivifyPlano, Texas-based remote monitoring startup Vivify Health (formerly Intuitive Health) raised $3 million, according to an SEC filing. This brings Vivify Health's total announced funding to at least $6.4 million to date.

MobiHealthNews reached out to Vivify Health for comment, but the company was not able to respond before deadline.

Existing Vivify Health investors include Ascension Health Ventures and Heritage Group. The latest round included two investors who contributed $1.5 million each.

Vivify Health offers a patient engagement platform targeted at hospitals, health plans, home health caregivers, and pharmaceutical companies. The company's platform includes a wide range of home monitoring features: coaching, customized care plans, video conferencing with physicians, and educational video content. The system integrates with remote monitoring technology like health and vital sign sensors, as well as with EHRs, PHRs, and HIEs, according to the company. Consumers can access the software through mobile devices, computers, and Internet-connected televisions.

Vivify has been quietly working on its software-as-a-service offering since CEO Eric Rock founded the company in 2009. The company relaunched as Vivify Health in May 2013.

Vivify has been deployed in large health systems since 2011, according to the company. Vivify has partnerships with AT&T, Ericsson, Polycom, and Samsung.

In August 2013, Vivify also partnered with vital signs monitoring company AirStrip to improve post-discharge home care and reduce 30-day readmission rates for cardiac care patients. The companies plan to develop a remote care information platform to share data.

“By supporting greater collaboration and accelerated response, we can transform home care in ways that improve patient outcomes while reducing costs,” AirStrip CEO Alan Portela said in a statement at the time. “This partnership will combine AirStrip’s mobile healthcare leadership position that enables faster, more informed clinical intervention throughout the care continuum with Vivify’s deep knowledge of patient engagement and population health.”
